Support Coordination Services

As your support coordinator I am privileged to play a pivotal role, walking alongside you as you achieve your goals.

The supports provided include;

✔️ Creating and reviewing your goals.

✔️ Help with understanding your NDIS plan - Your NDIS plan tells a story about you and your goals, how you will achieve them, and funding allocated for these.

✔️ Building your confidence and skills to understand and become involved in the use and coordination of your supports.

✔️ Budgeting and monitoring your funding where you will be provided a breakdown of your support costs and funding.

✔️ Maximise the use of your supports in your plan.

✔️ Explore and connect you with community, mainstream and NDIS services of your choice. Examples of these could include groups, activities, support people, health professionals and education. These collectively support you in achieving your goals, bring you joy and meaning to your life and help you maintain your overall well-being. 

✔️ Coordinate your supports.

✔️ Arrange care team gatherings with your supports and you. This can be extremely helpful with the reflection and progression of you achieving your goals. 

✔️ Plan review preparation such as revising your goals for the following year, gathering reports from Allied Health professionals, and writing assessment reports in support of your plan review.

✔️ Post plan implementation reports.

Service Level Agreement

When you choose to access Person-Centred Support, you will be provided with a Service Level Agreement to ensure a mutual understanding and agreement.

Cost for supports

The service costs are in line with the NDS Price Guide as per Support Coordination Level 2. Additional costs include travelling to you.
